Includes:
• Initial consultation visit
• Four companion visits with client and/or family
• Relationship-centered visits with the individual receiving care
• Caregiver support and education
• Vigil planning support
• Emotional and spiritual presence during transition period
• Phone/text support throughout service window
• Two grief support follow-up visits after passing
• Coordination support alongside hospice team (non-medical)
