• Initial consultation visit
• Six companion visits with client and family
• Ongoing relationship-centered visits with the individual receiving care
• Caregiver guidance and emotional support
• Vigil planning and bedside presence during active transition (as available)
• Legacy and remembrance support
• Advocacy support alongside hospice team (non-medical)
• Phone/text support throughout service period
• Three grief support follow-up visits after passing
• Personalized transition planning based on family wishes, culture, and values
